When the audit is completed but the same issues keep coming back: What is your organization missing?

Across many organizations within the automotive supply chain, internal audits are conducted regularly, audit plans are established, nonconformities are documented, and corrective actions are implemented. Yet after each audit cycle, familiar issues continue to resurface.

Production defects reoccur. Customer complaints persist. Corrective actions are closed, but their effectiveness is often not sustained over time. This raises an important concern: Is the audit process truly driving improvement, or is it simply confirming compliance?

According to Mr. Dinh Van Gioi, IATF Product Manager and Lead Auditor for IATF 16949, ISO 9001 and ISO 13485 at Bureau Veritas Certification, this is a common challenge faced by many manufacturing organizations implementing quality management systems in line with IATF 16949.

Audits Are Still Often Viewed as Compliance Checks

In practice, many internal audits remain focused on reviewing documents, records, and evidence to verify conformity with standard requirements. While this approach helps organizations assess compliance, it does not necessarily provide a true picture of process effectiveness.

A process may be fully documented yet still generate product defects. A corrective action may be formally closed in the system while the root cause remains unresolved. In such cases, the same issues are likely to reappear despite multiple audit cycles.

The Gap Between Audits and Operational Reality

Another challenge is that internal audits are sometimes conducted independently of actual operational data. Metrics such as defect rates, cost of poor quality, customer feedback, process performance, and recurring incident trends often reveal critical insights into the health of the management system.

When audits fail to leverage this information, organizations risk overlooking hidden issues that directly impact product quality and business performance.

An effective audit should help organizations address more fundamental questions:

  • Why does the same issue continue to recur?
  • Have corrective actions truly eliminated the root cause?
  • What risks exist but remain unidentified?
  • Is the management system effectively supporting quality and business objectives?

Answering these questions is what transforms auditing from a routine exercise into a meaningful driver of continual improvement.
